STRESZCZENIE
Do gabinetu trafiła 74-letnia pacjentka nosząca protezy od wielu lat. Jej życzeniem było poprawienie tak estetyki, jak i funkcjonalności protez. Wstępna ocena ujawniła różnice pomiędzy protezą całkowitą górną, starą i zniszczoną i całkowitą protezą dolną opartą na czterech implantach, która została wykonana niedawno. Aby uzyskać większą ilość przestrzeni dla nadbudowy implantologicznej, zęby zostały ustawione niewłaściwie, zajmując zbyt wiele miejsca. Doprowadziło to do nadmiernego ich wyeksponowania. Zdecydowano, w porozumieniu z pacjentką, o ponownym wykonaniu obu protez. Taka procedura gwarantuje optymalny efekt estetyczny umożliwiając jednocześnie całkowite odtworzenie funkcji żucia. Po podjęciu powyższej decyzji skoncentrowano się na wyborze odpowiedniego elastycznego połączenia pomiędzy implantami i protezą, tak aby zmieścić protezę wraz ze strukturą w dostępnej przestrzeni.

SUMMARY
The dentists meets a 74 years old patient who was carrying a prosthesis since many years. The patient’s request is to improve both the aesthetic and functionality of the prosthesis. First exam was showing a difference between the total upper prosthesis, old and damaged, and the total lower prosthesis, which was recently built over 4 implants. In order to assure more space to the implant’s structure the teeth were mounted improperly occupying too much space and resulting to be exposed. We decided to proceed in accordance with the patient by re-creating the total structure of the teeth both by the upper and lower level. This procedure will assure an optimal aesthetical result allowing a complete recover of the masticatory function. Right after these considerations we proceed in choosing the proper elastic connective system between the implants and the structure in order to assure the necessary thickness to set a correct prosthesis together with a structure and over structure suitable to the case.
